THE STUDY OF EXPECTED BLOOD UNIT IN THE ELECTIVE SURGERY




Abstract
Blood transfusion is indicated in the surgical patients to maintain circulating blood volume, adequate oxygenation and clotting factor level.
Anesthesiologists should be aware of the indication to transfuse and the volume of blood transfusion in order to carry out transfusion effectively.
To provide a preoperative standard of blood requirement in each elective surgery, we analyzed the number of elective surgery, number of transfused patients, number of transfused unit and mean of transfused blood per patient on the basis of the 4,962 anesthesia records of the surgical procedures during the period from January 1988 to June 1988.
The following results were obtained:
1) Number of patients for transfusion was 641 and it was 16.2% of the elective surgery and 12.9% of the total surgery.
2) Number of blood units for transfusion was 2,241 and mean blood unit for transfusion per patient was 3.5.
3) Number of transfusion with 1 unit was 84 and it occupied 13.1% of total transfusion.
4) Among the 45 kinds of operation, 15 kinds of operation (33%) were preoperatively considered to pre-pare the blood with 2--4units, whereas 2 kinds of operation with 69 units.
To reduce the abuse of blood transfusion, laboratory work loads, costs and to maintain the effective blood utilization, we recommend to prepare the preoperative blood requirement in accordance with the standard of expected blood unit in each elective surgery.
